I need a form on my website

Web Communications manages all forms through our online form tool, MachForms. Access to this tool is not granted outside of our department because of security concerns and governance/oversight needed for any data being gathered by an official University system.

If you are requesting a new form, open a help desk ticket with the following information:

  • Form Title

  • VUnetID of the Person Responsible for Form (if not yourself)

  • A list of the data you want to collect (e.g. Full Name, Department, Email Address, etc.)

  • What email address(es) should the form submissions be sent to?

  • Should a copy of the form submission be sent to the person filling out the form?  If so – what should the subject line be?

We will create the form using the options you provide and send you a preview to approve or submit changes.

Once the form is finalized, you have two options for how to use it on your website.

  1. Recommended: You can link straight to the form (no embed needed). We will provide you the URL to use.

  2. Optional: We can embed the form on a page on your website. Include the URL of the page you’d like to embed the form on in your original request.

If you need access to the submissions to the form as a spreadsheet, include that in your original request and we can provide you a unique URL through which you can view all submissions (these views can only be accessed if you are on the Vanderbilt network, or connected via VPN.)
