Web Forms

Digital Strategies manages forms through the form tools MachForm and Formstack. Access to these tools is granted by Digital Strategies in order to maintain security and oversight.

To request access to a form builder tool, open a Help Desk ticket and select the “Request Access to a WordPress Website or Web Application” request type.

There are two options for form use on a website:

1. Link directly to the form URL

2. Embed on a webpage
