Digital Strategies manages all forms through the form tool MachFormstools MachForm and Formstack. Access to this tool these tools is not granted outside of by Digital Strategies for in order to maintain security and governance/ oversight reasons.
To request a new form, open a Help Desk ticket with the following information:

Two options for form use:
Link directly to the form. Digital Strategies will provide the form URL.
Embed on a webpage. Include the URL of the page on which you would like the form.
If access to submission data is needed, include this in the Help Desk ticket request.
