Digital Strategies manages all forms through the form tool MachForms. Access to this tool is not granted outside of Digital Strategies for security and governance/oversight reasons.
To request a new form, open a Help Desk ticket with the following information:
Form Title
VUnetID of the Person Responsible for Form (if not yourself)
A list of the data you want to collect (e.g. Full Name, Department, Email Address, etc.)
Email address(es) where form submissions should be sent
If a copy of the form submission should be sent to the form submitter include the email subject line copy.
Digital Strategies will create the form using the options provided and send a preview for approval.
Two options for form use:
Link directly to the form. Digital Strategies will provide the form URL.
Embed on a webpage. Include the URL of the page on which you would like the form.
If access to submission data is needed, include this in the Help Desk ticket request.
