Digital Strategies manages all forms through the form tool MachForms. Access to this tool is not granted outside of Digital Strategies for security and governance/oversight reasons.

If you are requesting To request a new form, open a help desk Help Desk ticket with the following information:

  • Form Title

  • VUnetID of the Person Responsible for Form (if not yourself)

  • A list of the data you want to collect (e.g. Full Name, Department, Email Address, etc.)

  • What email Email address(es) where form submissions should the form submissions be sent to?

  • Should If a copy of the form submission should be sent to the person filling out the form ?  If so – what should the subject line be?

...

  • submitter include the email subject line copy.

Digital Strategies will create the form using the options provided and send a preview for approval.

There are two Two options for form use:

  1. You may link straight Link directly to the form (no embed needed). We . Digital Strategies will provide you the form URL to use.

  2. The form can be embedded Embed on a webpage. Include the URL of the page on which you would like the form.

If you need access to the submissions to the form as a spreadsheetsubmission data is needed, include that in your original request and we can provide you a unique URL through which you can view all submissions (these views can only be accessed if you are on the Vanderbilt network, or connected via VPN.)this in the Help Desk ticket request.
